THE SHORT VERSION

Keep requested details, confirmed sizes, the quote, and timing easy to distinguish. Use the form as an offline record and complete the actual order through the maker’s established process.

The original PDF is a one-page US Letter worksheet to print and fill in by hand. Print at Actual Size or 100%, then check that the full page is visible. The SVG lets you edit the layout in a compatible vector editor. The CSV opens as a field-by-field worksheet in a spreadsheet; make a separate copy for each request.

All fields are optional. Use only those needed for your request, and leave unknown details pending. The form has no submit button and sends no completed information to this website. It includes no payment fields, automatic calculations, or preset customer terms. The PDF is a print worksheet, not an interactive checkout form.

Describe the requested set before discussing extras

Start with a short request reference and the maker or shop. An optional name or handle can identify the conversation without putting a full address on a planning sheet. Record whether the quantity means complete sets, extra nails, or another item; a number alone leaves the contents unclear.

Write the shape, product-specific length, colors, finish, motifs, and where accent details belong. Link or name the reference image and give revisions a date or version. Separate the feature you want from elements the maker has not agreed to reproduce. A reference picture helps explain a request; it does not establish that the design, materials, or timing are available.

Etsy’s custom-item guidance recommends discussing the design specifics, cost, production time, and needed-by date with the seller. Use that same practical distinction on this worksheet: requested details stay requests until the maker confirms what can be supplied.

Tie all ten sizes to the exact product

Record the tip range, shape, and length before entering sizes. Use the maker’s requested measuring method or matching sizing samples, and identify that reference on the sheet. Enter left and right hands separately, from thumb through pinky. Do not copy one hand across because the design is symmetrical.

Mark whether the entries are measurements with units or size codes from the named range. A number without that context is ambiguous. Leave an unresolved finger blank and mark sizing pending instead of guessing a conversion. Use the separate sizing record if you need space for measurements, size codes, and fit notes together.

Record the intended attachment product or pre-applied system if it is known, and confirm what the set includes. This field identifies the product discussed; it does not establish compatibility or replace the product’s application and removal directions.

Separate the quote, requested date, and agreed timing

Record the quote currency, what it includes, and a reference to the complete quote. Note whether it is requested, received, still to confirm, or agreed. Keep missing extras visible rather than treating a starting price as a complete order total.

Write the requested need-by date separately from the dispatch or pickup date actually agreed with the maker. Keep an arrival estimate and its source on their own line. Etsy explains that estimated delivery combines preparation and carrier transit; a production period alone is not an arrival date. The worksheet supplies no turnaround promise.

Keep the current version easy to find

  1. Compare the design reference, product options, and ten size entries with the details discussed with the maker.
  2. Record the quote and timing confirmation reference, leaving unresolved items clearly marked.
  3. Date changes and keep one current version so an older image or size list is not mistaken for the final request.
  4. Use the maker’s established checkout or ordering channel when ready. For an Etsy purchase, complete the transaction on Etsy.
  5. Once the set is confirmed, carry its reference onto the packaging record and retain the exact product directions with it.

Keep the record small and private

The FTC advises businesses to collect and retain only information they need and protect what they keep. This worksheet asks for no card numbers, passwords, identity documents, or medical history. Use a request reference in place of extra identifying details where practical.

Keep completed files and paper copies away from public displays and shared photo folders. If delivery details are needed later, provide them through the maker’s appropriate ordering process. Share a blank template when someone asks for the form, not a previous customer’s completed record.

A few common questions

Can I edit this press-on nail order form template?

Yes. Edit the SVG layout in a compatible vector editor or fill in the CSV worksheet. Use a separate copy for each request. The PDF is designed to print and complete by hand.

Does completing the form send an order?

No. The downloaded files do not submit information to this website. Share only the needed details with your chosen maker and follow their actual ordering process.

Is the order form a display card or sizing chart?

It records a request, including sizes already checked using the maker’s method. Use the separate guides for mounting cards, measuring nails, and detailed fit records.
